The Community Care Coordination Program Manager is responsible for managing South Country's internal and delegate care management systems that are designed to ensure care is coordinated across services and settings for members.
This position is responsible for managing staff, responding to escalated operational issues/concerns, developing policies, and monitoring and evaluating the effectiveness of the organization's care management program.
Essential Accountabilities:
1.
Oversees and manages daily functions of care management teams, identifies and develops training and development opportunities to ensure compliance with contractual and regulatory requirements, reviews and evaluates work, develops and assists staff in goal setting.
Serves as a resource for staff members.
2.
Manages activities for the care management programs, including collaborating and coordinating activities with internal South Country departments, external partners, provider groups and other organizations that provide member care and services.
3.
Develops and evaluates quality assurance goals and objectives of care management programs for effectiveness, outcome measurement, identification of improvement opportunities and to ensure that programs are meeting regulatory requirements at the state and federal level, i.
e.
, Elderly Waiver, the National Committee for Quality Assurance (NCQA), Centers for Medicare & Medicaid Services (CMS), Minnesota Department of Human Services (MN DHS) and Minnesota Department of Health (MDH).
4.
Manages the oversight of and works with county partners in the areas of care coordination/case management to assure enrollees receive appropriate care according to quality and regulatory standards.
This includes leading and participating in various workgroups.
5.
Develops and revises policies, procedures and resources for care coordination programs as well as provide training related to policy & procedural changes.
6.
Leads the development of training programs to meet the identified educational needs of internal and external teams, i.
e.
, departmental policies and procedures, care coordination and care plan development.
7.
Represents South Country at external meetings and events with community partners and regulators, such as the MN DHS, MDH and CMS.
8.
Manages South Country's Model of Care and care coordination regulatory reporting requirements.
9.
Participates in departmental and/or corporate strategic planning designed to identify opportunities for new care management programs and/or improve efficiencies within care management programs.
10.

Financial Impact:
This position is responsible for ensuring that South Country is following contract language as promulgated by governmental agencies.
Failure to comply with this contractual language could result in breach of contract resulting in a significant financial penalty, potential contract cancellation, sanctions and/or corrective action.
This position has the responsibility to ensure enrollees receive timely, efficient care.
